Most people already receive services in their home or community from Fraser Health before requiring assisted living or long term care. If you are receiving service and have a community case manager, please speak with them about your further care needs.
You may also contact the Home Health Service Line at 1-855-412-2121 (open 7 days a week, 8:30 a.m. - 9:00 p.m.) to request an assessment. A community case manager will come to your home and complete an assessment to determine care needs.
- Home and community care
We provide a range of health care services to support people at home and in the community.
- Family guide to services for seniors
A what, where, and how to access guide to Fraser Health and community-based services to help seniors remain independent.
- Assisted living
Personal care and hospitality services for adults who can live independently. Residents live in apartment units furnished with their own furniture and belongings. They maintain a great deal of independence and privacy, while being supported with services.
- Long term care
We provide subsidized long term care for adults who can no longer live safely or independently at home due to their need for 24-hour nursing supervision in a secure environment, and their unpredictable or high-intensity care needs which can no longer be managed in their home setting.
